Online Eligibility and Claim Status


Due to the Change Healthcare cyberattack, these services are currently unavailable.  SAMBA is working to restore these functions as quickly as possible.

Electronic Funds Transfer


To eliminate paper checks and receive Electronic Funds Transfer (EFT) payments, please click here to complete the SAMBA EFT Authorization Agreement Form.

 

Electronic Remittance Advices


To receive Electronic Remittance Advices (ERAs), please register with Availity and then enroll to receive ERAs from SAMBA. Please click here to reach the Availity website and sign up for an account.

Precertification


The SAMBA Health Benefit Plan requires precertification for a number of different services including inpatient hospital stays. Please click here for more information.
