Identification Card
All members, including covered dependents, will receive a SAMBA Identification (ID) Card when they enroll. This card serves as both your medical and drug program ID card. Show your SAMBA ID card whenever you receive services from a Plan provider or fill a prescription at a Plan pharmacy.
If you did not receive your ID card(s) or if you need a replacement, call us at 800-638-6589 or 301-984-1440 (for TTY, use 301-984-4155). You may also request replacement cards via secure email.
