Online Eligibility and Claim Status

Health Care Professionals can check eligibility and view claim status online through our partnership with Change Healthcare.  

To accesss the SAMBA WebConnect portal, health care professionals must have a Change Healthcare User ID and Password. Please click here to Log In/Register.

Electronic Funds Transfer

To eliminate paper checks and receive Electronic Funds Transfer (EFT) payments, please click here to complete the SAMBA EFT Authorization Agreement Form.

Electronic Remittance Advices

To receive Electronic Remittance Advices through our partnership with Change Healthcare, please click here to complete the ERA Provider Information Form.  The SAMBA Payer ID is 37259.

Note: If you utilize a practice management vendor, contact your vendor for instruction on how to receive ERAs for SAMBA via Change Healthcare.

Precertification
